TRANSITION
The transition to secondary schools of SEN information for individual children takes place in the summer term.
The SENCO of each primary school historically meets with the SENCO of the secondary school that the child is moving to. (Covid regulations have meant this has been done electronically for the years 2020 and 2021) and a brief overview is shared. This will include what sort of support the child receives in primary school and the resources and interventions that are successful. If there is a file with information around your child's SEN needs, this will also be passed to the SENCO and will no longer be held at the primary school.
Each year ALL Year 6 children are well supported around transition. This comes from Trailblazers (at Bromley Wellbeing), via the secondary schools who visit us and meet the children and transition visits to the secondary schools; vulnerable or very anxious children are often offered further visits too. Some children with SEN needs have a pupil passport, although this is mainly in the form of a Pupil Support Agreement.
Transition to the next year's class within primary school is also something we work hard on. Each child is provided with the opportunity to spend some time in their new classroom, with their new teacher and class. A photo sheet of who is in their class is provided for the child to take home. This sheet will also have information on it relating to where they will hang their coats, where they will line up and even which group of toilets they will use. For more vulnerable and anxious children we will do a 'Moving Up' booklet for them to bring home and share with you. All of this is to aid their anxiety around the change of class.
